Fibrinous Acute Anterior Uveitis as a Manifestation of Poorly Controlled Type 2 Diabetes Mellitus

A male patient with diabetes presented with acute painful red eye associated with diminution of vision in the left eye. Ocular examination revealed severe fibrinous anterior uveitis: a dense anterior chamber reaction, with posterior synechiae. Infectious and autoimmune etiologies were excluded with thorough investigations, and uncontrolled hyperglycemia was attributed as likely precipitating factor. The patient responded rapidly to intensive topical corticosteroids and optimization of systemic diabetic therapy. This case highlights the association of poorly controlled diabetes mellitus with severe intraocular inflammation and emphasizes the importance of clinicians considering diabetes mellitus-associated anterior uveitis in patients with significant metabolic dysregulation.
